Shoth-Gorag is a creature from the Far Realm. It was summoned to Syra by Tephra to kill the Challengers of the Unspeakable so that she could retrieve the soul gems of Acererak from their corpses. However, Shoth-Gorag was itself slain by the Challengers; its body dissolved, leaving only its heart, from which it could regenerate in time. Rather than destroying it, the Challengers instead brought it to Myra Brigg at the Trine Academy in Silverstone in order to use it in a ritual to close Tephra's gateway to the Far Realm. The heart was placed in a secured vault, guarded by a variety of wards in case Shoth-Gorag began to regenerate.

This fear proved unfounded during the Battle of Silverstone, when the presence of so much aberrant energy resulted in Shoth-Gorag regenerating almost instantaneously. Myra and Harthur Grai were able to slay it once again with the aid of the wards and, unwilling to risk it being reborn a third time, Harthur shattered its heart with Farbane.
